Understudy Submission Form

At the completion of an understudy please submit a brief description of your experience as well as any feedback you received from the trainer. Make sure to highlight something you learned from each aspect of the Learning Connection Model (People Skills, Teaching Skills and Technical skills). Also be sure to include the clinicians name and the topic of the training. Submissions should be in pdf format in 500 words or less. You should take the time to debrief with the clinician at the end of training (30 min or less).
